Blizzard affirms it had "a few hundred thousand of D2R Items players in one region alone"--which could be quite a lot or fairly standard, depending on how Blizzard's servers define regions. (A few hundred thousand players would be impressive for the example of Australia.
For the definition of a "region" such as the United States, not so much, but if that is a tiny part that was part of United States, then maybe it could be. The blog post does not specify this. )According to the developers.
One of the major issues causing all of this is how Diablo 2 handles core pieces of player behavior. Even though Vicarious Visions updated the original D2 code as they could, the biggest portion of the development was keeping what code worked.

Diablo 2 has a particular method of pulling information in the central database in order to ensure that players can perform the things they'd like to accomplish. Joining a game? This is calling back to Central Database.
You'd like to switch your characters? That's another check to central command to ensure that you get the character would like, in the same spot you left it with everything you'd worked for buy diablo 2 resurrected items.
